League City Elite Dentistry In Office Membership Plan is a discount plan, and not an insurance plan. Membership fee is due at time of enrollment, is a one year contract, and is non-refundable nor transferable. Members will not receive a membership card. This plan’s effective date will be on file with our office. Treatment started prior to enrollment is not eligible for discount.

This is an in-office discount plan and is not a substitute for a comprehensive dental insurance plan. Membership cannot be combined with dental insurance nor can be applied to costs or services covered under workers compensation, or covered under auto or medical insurance.

For orthodontics and extensive treatment plans, member must remain a plan member for the duration of treatment to retain discount plan benefits.

Should member choose to use outside financing through Care Credit or Lending Club for dental treatment, the treatment discount will be reduced to 10% due to merchant fees.

Membership can only be used for services performed in our office. It is the sole responsibility of the member to maximize benefits by arranging the appropriate appointments within the membership period. If the benefits are not used, the member will not be entitled to a refund.
